Getting Patient Information on your Angiograms

Our Patient/Practice info labels are generated at the time we receive your angiograms, before we process your film. Therefore, if you want to be sure that Patient Name and Date Information get on the FA ID stickers, it is essential that we receive the information before we process your film.

Either:

1. Put a sticker on each roll with patient name and test date, or,

2. Put a number on each roll (with a permanent marker, please), and provide us with a list matching the roll numbers with the patient information



A few reminders:

PLEASE PRINT OR TYPE THE NAMES LEGIBLY especially when using little tiny stickers!
We also recommend you begin each roll by photographing the patient name (at the same flash power as for red-free photos, usually 25 or 36ws). This makes it easier to ensure that the film is yours.

ALWAYS USE OUR POLYVINYL RIPPROOF ENVELOPES, even inside FedEx, UPS or Priority Mail envelopes, to ensure that the canisters are not punched out in transit.



Getting ID information on your Color Slide Boxes

If you want each roll you send us to have unique ID information (which we recommend strongly), please do one of the following:

Either:

1. Put a sticker on each roll with ID info (roll #, first patient name, etc)

2. Put a number on each roll (with a permanent marker, please), and provide us with a list matching the roll numbers with the information.

We will put the ID info either on the label on the slide box, or (if you send a sheet) inside the box.

Unlike Justice, we do not provide a log sheet for each shipment to us. If you like their system, we recommend you copy a blank Justice sheet and use that. Or make your own.

We can also provide you with stickers to put on each roll that you can put the ID info on yourself.
